Nurse Practitioner

1. Position Overview

TandemStride is seeking an Ohio-licensed Nurse Practitioner to support the implementation of TandemCare—the clinical telehealth component of TandemStride’s trauma recovery model—by contributing to clinical process creation, workflow development, and daily patient care.

TandemStride supports trauma survivors through a technology-enabled model that combines app-based recovery guidance, peer support, community health worker support, and clinical care coordination. TandemCare focuses on helping trauma survivors transition from acute care into the first six months of recovery.

This is a unique opportunity for a Nurse Practitioner who wants to provide patient-centered telehealth care within the growing clinical program of TandemStride’s broader trauma recovery model.

The patient population may include survivors of spinal cord injury, traumatic amputation, internal organ injury, traumatic brain injury, polytrauma, major fractures, burns, and other serious traumatic injuries.

The ideal candidate will be an experienced Nurse Practitioner with a strong background in trauma care, primary care, or complex care management, combined with excellent clinical communication skills and a passion for improving long-term health outcomes. This role bridges advanced clinical care, multidisciplinary coordination, and patient advocacy with clinical rigor, empathy, and accountability—playing a critical part in reducing barriers to care and promoting whole-person well-being through technology-enabled, relationship-centered support. 2. Organizational & Reporting Structure

• Reports to: TandemCare Medical Director. Day to day administrative needs will be through the TandemStride Chief of Staff.
• Clinical collaboration: TandemGuides (community health workers) for community-based, non-clinical support
• Direct clinical reports: none. The NP does not supervise other clinicians; TandemGuides are managed operationally, not clinically, by the NP, though the NP sets clinical escalation criteria they follow.

3. Core Responsibilities3.1 Direct Clinical Care (Telehealth)

• Conduct telehealth assessments with trauma survivors during the post-acute recovery period.
• Support transition of care from hospital discharge into home and community recovery.
• Review discharge instructions, follow-up needs, medications, symptoms, red flags, recovery barriers, and patient concerns.
• Provide patient and family education.
• Identify gaps in care and help connect patients to appropriate providers and resources.
• Work closely with TandemGuides, who support patients as community health workers.
• Help define clinical red flags, escalation pathways, documentation templates, and communication workflows.
• Identify and escalate clinical red flags — readmission risk, wound complications, neurological changes, mental health crisis — to appropriate emergency or specialty care.
• Recognize when telehealth is insufficient and a patient requires in-person, urgent, emergency, specialty, or primary care follow-up.
• Document clinical encounters to a standard sufficient for payer audit, quality review, and continuity of care.

3.2 Transition-of-Care Ownership

• Own the clinical thread of the member's journey from acute-care discharge through the full 6-month TandemCare episode.
• Coordinate directly with hospital/discharge planning teams where relationships exist, to reduce gaps between acute discharge and first TandemCare contact.
• Partner with TandemGuides to align clinical follow-up with social/community barriers (housing, transportation, food insecurity) identified during triage.

3.3 Care Team Coordination

• Help set and communicate clinical thresholds that trigger TandemGuide outreach versus NP involvement versus emergency escalation.
• Participate in regular care-team check-ins with TandemGuides on shared caseloads.
• Coordinate with the peer support match on any member for whom clinical and lived-experience support intersect (e.g., a member disengaging from care for reasons that are partly clinical, partly psychosocial).
• Coordinate with health plan care management teams on shared members — exchanging status updates, aligning on care plans, and flagging TandemCare members who may need plan-level case management, benefits navigation, or authorization support.

4. Scope of Clinical Practice

The NP's clinical scope is bounded as follows:

• In scope: assessment, triage, care planning, medication management within SCA-authorized prescriptive authority, referral, and clinical escalation — all delivered via telehealth.
• Out of scope: any service requiring in-person, hands-on physical examination or procedure; day-to-day operational management of TandemGuides.
• Emergency scope: the NP is not an emergency service. Acute emergencies are directed to 911 / emergency department; the NP's role is early identification and warm handoff, not emergency management.

5. Required Qualifications

• Active, unrestricted Ohio Certified Nurse Practitioner (CNP) license, or ability to obtain one prior to start date.
• Board certification as a Nurse Practitioner (FNP, AGACNP, AGPCNP, or comparable — adult/family scope appropriate to trauma recovery populations).
• Physically located in Ohio.
• Willingness to enter into a Standard Care Arrangement with TandemStride's Collaborating Physician, as required under Ohio law (Section 4723.431), prior to beginning clinical practice.
• Demonstrated telehealth clinical experience.
• Ability to provide telehealth services to patients located in Ohio.
• Experience caring for medically complex patients.
• Excellent patient, family, and team communication skills.
• Strong clinical judgment and understanding of telehealth limitations.
• Comfort working independently in a remote environment.
• Comfort with ambiguity and iterative process; ability to document and improve a workflow while actively using it.
• Strong written documentation skills, given the fully remote, asynchronous nature of care team coordination.

5.1 Preferred Qualifications

• Prior experience in Medicaid managed care populations or value-based care models.
• Experience working alongside community health workers or peer support programs.
• Experience with medically complex, high-acuity, or trauma-recovery populations (SCI, amputation, TBI, polytrauma, burns, or similar).
• Background in rehabilitation medicine, trauma surgery follow-up, physiatry, or behavioral health comorbid with physical trauma.
• Start-up, small-team, or growth-stage health services experience.

6. Work Environment & Logistics

• Fully remote position; must reside in Ohio and hold an active Ohio CNP license.
• 100% telehealth clinical delivery — no in-person patient contact is part of this role.
• Requires reliable home office setup: private, HIPAA-compliant space for telehealth visits; secure, high-speed internet.
• Schedule structured around tiered visit cadence rather than a fixed daily census — expect variable day-to-day volume as the program ramps and as members move between tiers.
• Standard business software and telehealth platform training provided; workflow tools are still being finalized and the NP will help shape them.

Pay: $75,000.00 - $90,000.00 per year
