Licensed Clinical Social Worker (LISW or LPCC)

The Licensed Social Worker (LISW or LPCC) provides telehealth-based mental health services, including assessments, therapeutic interventions, and case management. This role focuses on client-centered care through virtual sessions and care coordination, with some collaboration alongside Community Health Workers (CHWs) who assist with client outreach and resource navigation. The LISW/LPCC ensures high-quality, accessible mental health support to clients, addressing both behavioral health needs and social factors affecting well-being.

Essential Job Functions: Clinical Assessment and Therapeutic Services

  • Conduct thorough mental health assessments, developing individualized treatment plans based on psychosocial evaluations.
  • Provide therapeutic interventions, such as cognitive-behavioral therapy (CBT), motivational interviewing, and crisis intervention, using a secure telehealth platform.
  • Adjust care plans as necessary, tracking progress and making modifications to support clients in achieving their mental health goals.

Care Coordination and Collaboration

  • Coordinate care with other healthcare providers, including primary care physicians, specialists, and other mental health professionals, to ensure integrated and seamless care.
  • Work with CHWs to support client needs related to Social Determinants of Health (SDoH), connecting clients with appropriate community resources for housing, food assistance, transportation, and financial support as needed.
  • Provide guidance to CHWs on when to escalate cases that may require clinical oversight or higher-level care.

Client Engagement and Education

  • Build strong, therapeutic relationships with clients, fostering a supportive virtual environment for mental health care.
  • Educate clients on coping strategies, self-care techniques, and available community resources to promote mental health and wellness.
  • Encourage clients to set and pursue realistic, achievable goals, offering regular support and progress check-ins.

Documentation and Compliance

  • Maintain timely and accurate records of all client interactions, assessments, and treatment plans in the electronic health record (EHR) system, following organizational and regulatory guidelines (e.g., HIPAA).
  • Oversee documentation by CHWs related to social needs and resource referrals to ensure comprehensive, coordinated care.
  • Participate in audits, case reviews, and quality improvement initiatives to uphold clinical standards.

Professional Development and Team Collaboration

  • Engage in ongoing training, supervision, and professional development to stay current with best practices in telehealth and behavioral health.
  • Participate in team meetings, case consultations, and collaborative care discussions as part of a multidisciplinary team.
  • Contribute to a positive, team-oriented work environment, sharing insights and strategies for enhancing virtual care delivery.

Other Duties as Assigned

Required Qualifications

  • Education: Master’s degree in Social Work (MSW) from an accredited institution.
  • Licensure: Active Licensed (LISW or LPCC) in Ohio, with eligibility for licensure in additional states if needed.
  • Experience: Minimum of 3 years of clinical experience in mental health, with telehealth experience preferred. Familiarity with addressing Social Determinants of Health and working in a multidisciplinary setting is advantageous. Experience working with clients with PTSD is a plus.

  • Clinical Skills: Expertise in assessment, diagnosis, and treatment planning, using evidence-based approaches.
  • Communication: Excellent verbal and written communication skills, especially in a virtual work environment.
  • Organizational Skills: Strong ability to manage time and documentation for a caseload of clients, including collaboration with CHWs.
  • Technical Proficiency: Comfort with telehealth platforms, EHR systems, and video conferencing tools. Experience with Google Workplace, Slack and Microsoft Office suite.

Technical and Workspace Requirements

  • Internet Connection: High-speed, reliable internet connection for virtual sessions and remote team collaboration.
  • Workspace: Private, quiet workspace to ensure confidentiality in telehealth sessions.
  • Technology: Familiarity with EHR systems, video conferencing, and Google Workspace for remote work.

Work Environment and Hours

  • Remote Work: This is a fully remote position, requiring independence and effective self-management. This may evolve into onsite, hybrid work.
  • Flexible Schedule: Standard hours with some flexibility based on client needs, including potential evening or weekend availability.
