About Self Fund Health
Self Fund Health is a Wisconsin startup helping Midwest employers do something radical: buy healthcare instead of health insurance. For companies with 50+ employees, we pair transparent, upfront pricing with a clinical team of Nurse Navigators who guide members to high-value care — bending the cost curve in a state that ranks among the highest in the country for healthcare costs. We're a small, fast-moving team where everyone has a real hand in rebuilding the health plan from the ground up, and where the work directly lowers costs for local businesses while improving care for the people who count on them.

Position Summary
Your primary mission is to iterate on our growing fleet of LLM agents and tooling. Day-to-day, that means executing the end-to-end engineering: you’ll build agent logic, tools, and prompt structures, integrate APIs and MCP servers, dive deep into conversations logs to spot areas for improvement.
Self Fund Health is betting heavily on practical AI agents to bend the administrative and clinical cost curve in healthcare. You’ll ship production-level LLM-based, human-in-the-loop tools and the software. That means you’ll need a keen understanding of the healthcare industry problems we’re solving, the ability to use LLM-assisted development tools and practices to efficiently create releasable software, awareness of the limits and risks of the technology we’re using, and consideration of how humans will interact with these tools through their lifecycle.
But “softwave engineer” in our startup environment isn’t limited to just coding and shipping agents. You’ll be collaborating with the small but mighty technology team across the stack to ensure the stability, security, and maintainability of the tools that run the company. This might mean contributing to our internal database and API/MCP tools layers, sharing best practices for local environment setup, tweaking website design and copy with our marketing team, or advising on how to configure a phone tree such that it creates automation opportunities.
Key Responsibilities
- Design and Ship AI Agents: Build production-grade agents and multi-agent workflows on modern agent development frameworks (Google Cloud / Google Agent Developer Kit).
- Curate Agent Tooling & Data Feeds: Take ownership of creating and improving APIs, data pipelines, search tools, and database connectors that feed context and execution power into our agent fleet.
- Deep Healthcare Domain Grounding: Translate complex health plan concepts—plan designs, network tiers, copay structures, claims data, and clinical encounter feeds—ensuring agents operate within realistic bounds.
- Evaluation, Guardrails & Reliability: Review and iterate on production activity of agents, build evaluation and measurement into the development process, and participate in architecting our technology for reliability and security.
- Cross-Functional Collaboration: Partner closely with fellow technologists, clinical teams, operations, and sales to automate and improve upon loose operational pain points.
Required Qualifications
- Mid-career software engineering experience. You have a proven track record of independently shipping within production systems, and a sense of architectural tradeoffs within those systems.
- High proficiently with modern LLM-based development and agentic systems. You have built real systems using LLMs, structured outputs, function/tool calling, retrieval systems, and agentic workflows (familiarity with Google Cloud AI ecosystems, Agent Development Kits, or equivalent frameworks).
- Proficient in leveraging LLM coding assistants. These tools (e.g. Claude Code, Antigravity) multiply your skill, letting you ship clean, production-grade code rapidly without abstracting away your judgement.
- Healthcare domain literacy. You understand the fundamental mechanics of health plans, self-funding, medical claims, eligibility, provider networks, or digital health data.
- Comfort with ambiguity. You can take an ambiguous challenge ("our clinical team needs an assistant that cross-references provider network costs with member plan designs") and turn it into working architecture.
Preferred Qualifications
- Residency in the Upper Midwest (WI, MN, IA, IL) is strongly preferred for occasional opportunities (a handful per year) to engage with the larger team, clients, and partners, though the role is remote.
Skills and Competencies
- Pragmatic builder. You write code that ships. You care more about solving the business problem reliably than over-engineering academic abstractions.
- Analytical instinct. Healthcare data is notoriously messy. You have a knack for spotting data anomalies and structuring disparate health data.
- Domain curiosity & empathy. You understand that behind every claim and plan rule is a real person trying to get care. You build systems that hold up to the human stakes of healthcare.
- Self-starter. You don’t wait for a detailed Jira ticket. You talk to the people doing the work, figure out the bottlenecks, and build the solution.
- Adaptable. You thrive in a dynamic environment where company priorities evolve and you have the autonomy to help define the technical playbook as we grow.
Salary & Benefits
- Salary: $100,000 - $125,000 per year
- Health benefits: Coverage under Self Fund Health’s own plan, including direct primary care access at no cost. Dental insurance.
- Retirement: 401(k) plan.
- Paid time off: Unlimited PTO.
How to Apply
Submit your resume and a thoughtful answer to the question “Briefly describe your ideal version of how LLMs and agentic systems are part of this role.”
The interview process will consist of a 30 minute introductory call, one hour technical session, a 30 minute cross functional call, and a 30 minute peer call.
